1. I need an email/computer account created for a new employee... How do I do that?
You can use the "Computer/Email User Account Request" online form to request an account for a new employee. The online form can be filled out by the PD, Supervisor, Office Manager or the employee themselves (with help from a Supervisor). The form will walk you through providing information just as name, job title, email group requirements, start date, etc.
We encourage you to fill out the form ahead of the employee's start date. This way the account will be created and ready to use the day the employee starts.
After the account is created, a welcoming email will be sent to the employee and another email will be sent to the PD and supervisor indicating that the account has been created.

2. If an employee will be transferred to another facility, please use the online "Employee Transfer Form" This form should be used in one of three different scenarios:
a. if employee is being transferred from one facility to another
b. if employee is being added to an additional cost center or facility
c. If employee is being removed from a cost center or facility
You will be asked for employee information, what email groups the employee needs to be added to or removed from and other information. Once the form is submitted to IT, this information is used to add/delete the employee from the proper security and email groups.. You will be notified once the transfer is complete.

3. I need to terminate an employee. What do I need to do to disable his account and make sure proper departments are notified?
Use the "Workforce Termination Form" if a workforce member needs to be terminated. Here are the steps to fill out this form:
a. Fill out the form with all pertinent information. When you click "Submit", an email will be sent to your CRF inbox.
b. check your email and click the verification link in the email. This will take you to the "confirmation" page to confirm the information you just entered.
c. Confirm at the bottom of the page. The information will be sent to the proper departments for processing (HR, Admin, Accounting, IT, etc.)
d. IMPORTANT: Because of the security mechanisms included in the Termination form you must complete steps a through c all on the same computer and within a 24 hour period (or you will have to start over). You also must do one termination at a time. If you have multiple terms to do, do steps a through c for the first employee... once complete, then you can start with step a for the second employee.
